TRAVEL RELEASE

   When a student has a sufficient need to justify transportation to or from an athletic event by a parent, the following form must be completed and on file in the Athletic Director’s office 48 HOURS PRIOR  to the day of the contest or if a Saturday contest, by the end of the day on Thursday.

It is my understanding that the Athletic rules of Tinley Park High School require students to ride buses/school transportation to and from all athletic events and a departure from this requirement will release Tinley Park High School , District 228 from all liability for any adverse results that may occur.

I agree to release Tinley Park High School , District 228, and its employees and the Board of Education members from all liability with reference to the above stated transportation.
