| New Entrants/Transfer Students Requirments: |
New or transfer students need to provide the following documentations
to register as a student.
- Birth Certificate
- Proof of Residency:
-Phone Bill
-Contract to Buy or Signed Lease
-Current Pay Stub
- Proof of Custody
If student does not live with both parents. Must have paper filed in
court showing District #228 parent with sole or residential custody.
Or guardian must have paper filed in court to show Legal Guardianship.
- For Shared Residence:
-Notarized Shared Residence Form
-Phone Bill of person with whom you are sharing residence
-In 30 days you will need to provide proof of your Oak Forest residence
-- pay stub or utility bill
- Test Scores or Transcripts if transferring from another school.
- ISBE 33-78 Form (if coming from another IL Public School)
