School Request Form
During this
experience, you will be working with a teacher in a elementary or middle
school in the
Oak Forest
area. You have the opportunity to ask for a particular school,
teacher, grade, and subject. The goal is to place you in a situation
where you are most comfortable and knowledgeable in order to have success.
By filling out this form, it will give you a better chance to have your
ideal situation.
As your placement moves forward, the coordinator
will be putting the subject area as a first priority (particularly with
middle school requests). The second priority is the grade level.
Finally, the coordinator will be asking for the teacher the intern is
requesting. Everything will be worked out through the building
principal. The principal will have the final decision concerning
the placements in his/her school.
